At Northern Lakes Equine Practice, our commitment to providing top-notch veterinary care remains unwavering. To ensure our continued delivery of exceptional veterinary care and customer service, we want to highlight key business policies for 2024.
1. Valid Veterinary Service Agreement (VSA) on File:
This form is required to maintain a healthy veterinary client patient relationship. It also allows us to update our files annually and ensure that our records are current. If your information (address, email, phone number, credit card, patient info, etc) has changed or you’ve never filled out a VSA, please click here.
2. Valid Credit Card on File:
We kindly request that all clients have a valid credit card on file. Per your VSA, you’ve selected one of the following and your credit card will be used accordingly:
- Enroll in AutoPay. Please charge the below listed credit card following each billing cycle and send me a paid receipt.
- Keep a credit card on file for emergency purposes only. Alternate payment will be made upon receipt of invoice, and the listed credit card will only be used if specified, or if a balance exists for any reason beyond 30 days of invoice.
This will streamline our billing process and contribute to the efficiency of our administrative operations. To update your credit card on file, please click here.
3. Payment Plans:
For larger veterinary services, we understand the financial commitment involved. If payment plans are required, we offer the following options:
- 2-3 equal payments: To set this up please contact the office (847-388-9141 x 5) prior to service.
- Scratch Pay: This service allows you to break up your balance into five equal biweekly payments. To learn more, please click here.
- CareCredit: For financing up to six months, we recommend utilizing CareCredit, a trusted financing option that can help you manage your veterinary expenses more effectively. To learn more, please click here.
4. Change of Ownership/Leasing:
NLEP will continue to bill the owner on file unless we are notified otherwise. Please know that it is the owner’s responsibility to contact Northern Lakes Equine Practice regarding any changes in ownership or lease agreements. We are only able to change financial ownership after both parties have contacted our office to authorize billing adjustments. We are not able to make retroactive billing changes.
